Dexamethasone Antacids

The absorption of prednisone, and probably prednisolone, can be reduced by lai e but not small doses of aluminium/magnesium hydroxide antacids. Dexamethasone absorption is reduced by magnesium trisilicate. [Pg.1049]

Delavirdine is extensively metabolized to inactive metabolites by the CYP3A and CYP2D6 enzymes. However, it also inhibits CYP3 A and thus inhibits its own metabolism. In addition to its interactions with other antiretroviral agents (see Table 49 1), delavirdine will result in increased levels of numerous agents (Table 49-3). Dose reduction of indinavir and saquinavir should be considered if they are administered concurrently with delavirdine. Delavirdine plasma concentrations are reduced in the presence of antacids, phenytoin, phenobarbital, carbamazepine, rifabutin, and rifampin concentrations are increased during coadministration with clarithromycin, fluoxetine, dexamethasone, and ketoconazole. [Pg.1140]

Information seems to be limited to these studies. The indication is that large doses of some antacids can reduce the bioavailability of corticosteroids, but small doses do not, although this needs confirmation. One manufacturer of dexamethasone suggests that the doses of antacid should be spaced as far as possible from the dexamethasone, while another suggests an interval of at least 2 hours. In other similar antacid interactions 2 to 3 hours is usually sufficient. The manufacturers of deflazacort also suggest an interval of at least 2 hours between administration of deflazacort and antacids. " Concurrent use should be monitored to confirm that the therapeutic response is adequate. Information about the interaction of other corticosteroids and antacids is lacking. [Pg.1049]
