Traffic safety is very important in tunnel construction as many subcontractors work inside the same tunnel. To achieve an accident-free record, drivers have to attend KYT every day to remind them to restrict their speed to less than 5 km/h. Before ffiey enter the tunnel, they must shout don t speed . 

After a run of nearly two years accident-free, a company employee slipped on a step and was unlucky enough to fracture a small bone in his foot. He was unable to work for several weeks and an LTI was recorded with a subsequent loss of safety awards to staff. At about the same time a container was dropped during an off-shore lifting operation. This latter incident had tremendous potential for injury but as luck would have it no-one was hurt. There is no doubt that the lifting incident was much more serious. 

We will continue to see a continuation of the constant quests to protect crew, passengers, maintenance persoimel, third parties and the environment from the ever-present risks associated with operating and maintaining complex (and especially high kinetic energy) machinery such as aircraft. Although an accident-free society is an unrealistic dream, it can be tempered by technical excellence in design, maintenance and operation to continually improve on the safety record. 

Individual employees are paid 250 if the department works 1 million injury-free man-hours, so this is a motivator to drive accidents and the subsequent injury underground. For the 3.5 million record, a 5 g gold bar was given to all employees ... 

In order to define an accident database, we have to specify the types of facts or data elements to be included in each accident or near-accident record. We also have to specify a data type for each element. This tells us whether the data is coded according to a nominal, ordinal, interval or ratio scale or stored in free text.
