Workplaces fact-finding

Accident investigation should be fact-finding and not fault-finding and, if used effectively, can contribute greatly to the reduction of accidents and near miss incidents occurring in a workplace. Once the near miss incidents are identified and their causes corrected the probability of accidents occurring is reduced. 

Let s assume that the records you examined and the discussions you held indicate that a health problem exists somewhere in the plant and it seems to be chemical in origin. One of the first things you can do to find out what s causing the problem, and thereby recognize a need for further action, is to identify all the potentially hazardous materials in the working environment.
