In the ontological approach to medical diagnosis, each disease is thou t of as a distinct entity. An alternative approach to the diagnostic process is the physiological approach. Disease is defined in terms of objective manifestations. For example, the disease hypertension is based on a physiological measurement Hyperthyroidism is characterized by an increased rate of accumulation of radioiodine by the thyroid gland. Different diseases may have some of the same manifestations. Examples are senile dementia of the Alzheimer type (SDAT) and idiopathic Parkinson s disease. 

The types of tumour which cause a permanent hypertension, and which may be preceded by a stage of paroxysmal hypertension, frequently simulate essential hypertension or, in the course of a prolonged development, a malignant nephrosclerosis. Alterations in the vascularity of the retina, proteinuria, and an increasing reduction of the renal functions, are the consequences. The relatively frequent occurrence of such symptoms make it necessary to take the phaeochromocytoma into consideration as a possible cause of every hypertension of long duration. It may also be the cause of a diabetes resistance to insulin and accompanied by hypertension, or the simultaneous occurrence of hypertension and an increased basal metabolic rate together with a normal radio-iodine absorption by the thyroid gland. The illness is very improbable in the obese. 

The members of the sub-phylum Urochorda do not show the biochemical features we have described as being characteristic of Vertebrates but they have features of their own. They possess a cellulose coat and th accumulate vanadium in their tissues and blood cells. Although there is no thyroid secretion, they are sensitive to thyroxine which activates the development of the larvae. Also, their neural glands contain prindples similar to the hypertensive, melanophore-expanding, ox rtocic and gonadotrophic principles of the vertebrate hypophysis.
