Thrombolysis combined pharmacologic therapy

Ultrasound-assisted soft digestion has been used for other practical purposes from which analytical chemists can derive new applications. One case in point is in medicine, where ultrasound has been used as an adjunctive therapeutic treatment for clot dissolution of pharmacological thrombolysis. The combination of externally applied low-frequency high-intensity US with fibrinolytic therapy resulted in more rapid and complete reperfusion than the application of US or administration of fibrinolytic agents alone [42]. 

Initial fibrinolytic therapy (in patients who have no absolute contraindication to thrombolysis) should be combined with administration of conjunctive pharmacological agents to enhance lysis and minimize the risk of Weeding. 

We believe that pharmacoinvasive therapy has the potential to combine the best aspects of both pharmacologically and intervention-ally induced and sustained reperfusion (Fig. 1.8). Its potential should, of course, be the focus of debate and hence investigation. Contrasting the merits of thrombolysis with those of primary PCI should not. 

Refinements and improvements in pharmacologic ( thrombolysis ) and mechanical approaches ( primary angioplasty ) to recanalization have been impressive. Randomized clinical trials and registries have been used to compare and contrast the two, but assessment of both in combination lay dormant until quite recently. Improvements in coronary thrombolysis and coronary interventions now make reassessment of pharmacoinvasive therapy particularly desirable. Reassessment is indicated given the inherent limitations, when used alone, in both pharmacologic (failure to restore flow adequately) and mechanical approaches (unavoidable delay that can prolong the time to initiation of reperfusion).
