An overview of the main aspects of endothelial dysfunction as they relate to thrombogenesis and inflammation is important to provide a background of the factors affecting EMP release and their phenotype. A large number of reviews are available on this subject however, this section describes only the main features of EC perturbation and dysfunction as may be relevant to EMP.

Because most hot flashes resolve in 1 to 3 years, short-term use may be all that is needed, and can significantly improve quality of life. Short-term use, however, may increase the risk for cardiovascular events due to thrombogenesis and coronary heart disease, and should generally be avoided in women with preexisting cardiovascular disease, prior history of blood clots, or any condition that increases the risk for thrombotic events. With each year of use the risk for breast cancer and gallbladder disease increases. Long-term use decreases the risk for colon cancer and hip fracture. A semiempirical/theoretical ionic model was derived to cor-relate and interrelate the ultrastructure morphology, surface charge, surface chemistry, and surface molecular motions of a model semicrystalline hydrophobic triblock copolymer to thrombogenesis. This chapter addresses the aspects of ultra-structure order vs. disorder, primary and secondary molecular motions, surface and side chain chemistry, thrombogenesis, and the resultant ionic model. This model can be extrapolated to predict the relative thrombogenic responses of various crystalline and semicrystalline hydrophobic polymeric substrates. 

We, therefore, developed a new method for enhancing albumin adsorption, a method that may provide indefinite protection against thrombogenesis and cell adhesion. Marin et al. (M7) reported that patients with atrial fibrillation had lower levels of plasma MMP-1 but increased levels of TIMP-1 and prothrombin fragments FI 2 (an index of thrombogenesis) and higher ratios of TIMP-1 to MMP-1 as compared to control subjects. The authors proposed that MMP TIMP measurements in blood might be useful as markers of comorbidity that is associated with increased risk of stroke and thromboembolism in patients with atrial fibrillation.
