Separated plasma this is plasma separated from the red cells. Blood is centrifuged and the plasma is then sucked off and studied in isolation. 

True plasma this is plasma in equilibrium with its red cells. This nomenclature is intended to draw attention to the fact that the blood analysis yields concentrations in the plasma phase. To be accurate, whenever graphs have been drawn and have been said to indicate the behaviour of blood, in fact they have indicated the behaviour of true plasma, the plasma moiety of the whole blood. 

Measure PCO2 with blood gas meter pH with glass electrode. Calculate (HCO ) by Henderson-Hasselbalch equation.
