Talwin - Naloxone

It is worth mentioning that iV-allylic substitution in a number of morphine derivatives, as a rule, leads to antagonistic properties. Naloxone is a few times stronger than nalorphine as an antagonist. It blocks opiate receptors. It eliminates central and peripheral action of opioids, including respiratory depression. Naloxone is used upon overdose of narcotic analgesics.Synonyms for this drug are narkan, talwin, and others.

Talwin NX - Talwin NX tablets, which contain naloxone, produce analgesic effects when administered orally because naloxone has poor bioavailability. 

Hypersensitivity to pentazocine, naloxone (in Talwin NX) or any product component.

Tablets for oral use now contain pentazocine hydrochloride (equivalent to 50 mg of the base) and naloxone hydrochloride (equivalent to 0.5 mg of the base Talwin NX), which reduces the potential use of tablets as a source of injectable pentazocine. After oral ingestion, naloxone is destroyed rapidly by the liver however, if the material is dissolved and injected, the naloxone produces aversive effects in subjects dependent on opioids. An oral dose of about 50 mg pentazocine results in analgesia equivalent to that produced by 60 mg codeine oraUy.

Pentazocine (Talwin) Similar to morphine, but less potent. Antagonized by naloxone, not by nalorphine. Moderate pain. Also used as preoperative medication. Similar to morphine. 

The amount of naloxone present in Talwin Nx (0.5 mg per tablet) has no action when taken orally and does not interfere with the pharmacological action of pentazocine. However, this amount of naloxone given by injection has profound antagonistic action to the opioid analgesics.
