As a result of converting the data to a histogram it is mnch easier to see what the data looks like and see the range that the data covers. It becomes much easier to make decisions with the data in this more nsable format.

Accidents in A— record any accident number on the HBA tally sheet that appears four times in the A category.

After recording the accidents from rows A and C on the HBA Tally Sheets (see Tables 11.4 through 11.7), the data are ready to do an HBA. Go back to the accident abstract and for each accident, ask the questions to determine which human barrier was operating. 

When analyzing each accident, it puts one in a forced choice sitnation. Bnt only select one of the five possible human barriers as the cause of the accident. Using one s experience in the industry, and the knowledge of the industrial operations and procedures, decide which of the human barriers you think probably led to the performance and contribnted to the accident. Then place a checkmark in the column under the appropriate barrier for that accident. For each accident on the HBA Tally Sheet, only one checkmark can be in one of the five colnmns. 

After you have recorded the accidents from rows B and D on the HBA Tally Sheets, complete those forms by filling in the blanks with the barrier that was not provided and the barrier that failed, respectively. 

Check sheets or tally sheets—used to take the raw data and reorganize it into specific categories that are being observed. 

MSHA s 1988 Coal Accident Analysis and Problem Identification Instruction Guide shows how a system of classification designed to collect premium data can become an artificial template that structures not only the activity of the inspection but also the conclusions that inspectors can draw from their data. The Instruction Guide provides a set of checklists, tally sheets, and planned inquiry sheets to help inspectors categorize accidents. The classification system in the Physical Barrier Analysis Matrix (Fig. 3.1) seems to support the conclusion that physical barriers are either not possible or practical in most of the cases reported on the form—a conclusion that might exempt management from responsibility for protecting workers. The data that engineers collect may he raw or unorganized however, when dealing with a large set of raw data, it is helpful to group the data into various classes. The data could he grouped into class intervals to form a frequency distribution, which shows the number of observations that occur within a given interval. In fact, it may be possible to classify the data in this way with a tally sheet as they are collected (see Table 7.13).
