Note that in Table 9.12 the second most frequent OSHA citation issued for oil and gas drilling operations was for violation of 29 CFR 1910.146, the permit-required confined spaces standard. OSHA has a specific standard that mandates specific compliance with its requirements for making confined space 

Acceptable entry conditions— The conditions that must exist in a permit space to allow entry and to ensure that employees involved with a permit-required confined space entry can safely enter into and work within the space. 

Attendant— An individual stationed outside one or more permit spaces who monitors the authorized entrants and who performs all attendant duties assigned to the employer s permit space program. 

Authorized entrant— An employee who is authorized by the employer to enter a permit space.
