Patients receiving 5-ASA should be screened regularly in order to detect signs of renal impairment. It is suggested that serum creatinine concentration should be measured each month for the first 3 months of treatment, three monthly for the remainder of the first year and annually thereafter [26]. The use of concurrent immunosuppressive therapy may necessitate extension to the period of intensive monitoring. Moreover, it is shown that IBD patients with a stoma and patients with extreme dehydration are more susceptible to develop renal impairment (Table 3). 

Fifth Type.—The stomal mother-cell divides once or several times before becoming the true mother-cell of the stoma. As a result of the divisions there are also formed one or more subsidiary cells. This mode of development is seen in the Lahiatas, Papilio-nacem, Cruciferce, SolanacecB, Crassulacece, Cactacew, and Begornacea, also in a number of ferns. 

Investment on research and development of new materials will only be maximised if there is a clearly established route to market for newly developed products. In the UK, the routes to market for products used in the community setting have been well established for some thirty years. Recent efficiency drives initiated by the Commercial Directorate of the Department of Health threatened to disrupt the routes to market which traditionally have meant that the UK is a place of choice for research, production and the first launch of many new technologies. This paper explores the history behind the reimbursement process for appliances in the community setting, and the consultations aimed at reviewing tire arrangements for the provision of dressings, incontinence appliances, stoma appliances chemical reagents and other appliances to primary care and secondary care. Althou the process is not yet complete, the paper sets out to update on the current position and the implications to research and development. 

A 48-year-old woman developed a fever and pain in the right lower quadrant of the abdomen 8 hours after a barium study that was performed through an ileostomy stoma [19 ]. Her temperature was 38.5°C and there was tenderness and rebound tenderness over McBurney s point. The white cell count was raised. A supine plain abdominal X-ray showed retained barium in the cecum and appendix, which was confirmed by a Cr scan, which also showed mild swelling of the appendiceal walk The appendix was resected. It was red and edematous and there was barium in the lumen. 

Mouth. Cheilosis (sores around the mouth) develops.The lips become swollen, reddened, and chapped, and characteristic cracks develop at the corners of the mouth (angular stoma).
