Stimulant antidepressants

The few controlled studies of pharmacotherapy for AN have largely been disappointing. No class of medication has consistently proved effective in the treatment of AN consequently, pharmacotherapy plays a relatively minor role in the routine management of the disorder. Nevertheless, a review of the medications tested for the treatment of AN is informative. Medications used in the treatment of AN include appetite stimulants, antidepressants, antipsychotics, anxiolytics, trace mineral supplementation, prokinetics, and opiate antagonists. 

As noted above, Zito and colleagues (2000) reported stimulant use in a Medicaid population of 12.3/1000 preschoolers, with rates of stimulants, antidepressants, and other medications such as clonidine increasing dramatically since 1991. Although more stimulating antidepressants (e.g., bupropion, SSRIs, venlafaxine, or certain MAOIs) do not potentiate alcohol, they can produce insomnia. To minimize this problem, the dose may be given earlier in the day. TCAs may cause episodes of excitement (rare), confusion, or mania, usually in patients with an underlying psychotic illness, suggesting that a preexisting disorder must be present for these drugs to exert any psychotomimetic effects.

In 1995, CHADD also had smaller grants from Abbott Laboratories ( 37,000) and Burroughs Wellcome ( 18,000). Abbott is the manufacturer of the stimulant pemoline (Cylert), used to treat ADHD. Burroughs Wellcome makes several medications used in pediatric medicine, including well-known antibiotics and cold medications. They also make the highly stimulating antidepressant Wellbutrin. 

CBT alone is indicated, as it is associated with improvement in 80% to 90% of patients short-term and 75% of patients at 6-month foUow-up. Patients must be educated to avoid caffeine, drugs of abuse, and stimulants. Antidepressants, especially the SSRIs, are preferred in elderly patients and youth. The BZs are second line in these patients because of potential problems with disinhibition. 

Psychoactive substances (including plants) are those that alter some aspect of the mind including behavior, mood, anxiety, cognition, and well-being [61]. They may be classified in three types a) depressors of Central Nervous System (CNS) activity, such as neuroleptics, anxiolytics, and hypnotics b) CNS stimulants - antidepressives and amphetamines, mainly and, finally, c) disturbers of the CNS - hallucinogens [62].
