Pyrazinamide dosage

Pyrazinamide can cause pellagra. However, prophylactic nicotinamide is not generally recommended. In cases of pellagra, a dosage of 300 mg/day should be given (8).

Pyrazinamide is an important component of short-term (6-month) multiple-drug therapy of tuberculosis. The daily dose for adults is 15-30 mg/kg in a single oral dose. The maximum dose is 2 g/day regardless of weight. Children should receive 15-30 mg/kg/day daily doses also should not exceed 2 g. Pyrazinamide is safe and effective when administered twice or thrice weekly (at increased dosages). 

Pyrazinamide (PZA) is an important first-line antituberculosis drug and was discovered in 1940. It is used as a main dmg in the treatment of MDR TB (Singh et al., 2006). PZA reduces the TB treatment from 12 months down to 6 months and it is absorbed orally with a dosage of 80 mg/kg body weight (Wade, 2004). It is a derivative of nicotinic acid (Konno et al., 1967 Loeffler et al., 2012) (Figure 11.2). 

The test is performed by determining uric acid excretion before and after the administration of pyrazinamide or its metabolite pyrazinoic acid (PZA) in a dosage sufficient to give maximal suppression of uric acid excretion. The decrement in uric acid excretion after PZA administration is taken as a quantitative measure of uric acid secretion. In addition the difference between filtered load of uric acid and uric acid excretion at the time of maximal PZA effect is taken as a quantitative measure of uric acid reabsorption.
