Psychiatric adverse events

Azathioprine has been newly associated with psychiatric adverse events (80). 

The potential for severe psychiatric adverse events must always be considered in patients taking long-term chloroquine and hydroxychloroquinine. The onset may be a few hours to many days after the start of therapy and it can occur in a patient without a preceding history of mental illness. The mechanism is unknown. Recovery is rapid and occurs within days of stopping treatment. 

Psychiatric adverse events, most often hostility and irritability, have been reported in children taking levetiracetam. Four cases of acute psychosis have also been reported (448). 

The prevalence of psychiatric adverse events in relation to topiramate has been studied in 431 consecutive patients (648). There were psychiatric adverse events in 103 patients (24%), including affective disorder in 46 (11%),... 

Topiramate has been associated with several psychiatric adverse events, such as nervousness, depression, behavioral problems, mood lability, and psychosis. Obsessive-compulsive disorder has also been reported (650).

The incidence of rash was very low (0.3% with placebo, 0.5% with 400 mg, 1.1% with 800 mg, and 3.2% with 1200 mg of eslicarbazepine). Hyponatremia (< 125 mmol/1) was reported in four patients, and the incidence of behavioral or psychiatric adverse events was <1%. The overall incidence of adverse reactions leading to withdrawal of therapy was 4.5% with placebo, 8.7% with 400 mg, 12% with 800 mg, and 19% with 1200 mg of eslicarbazepine. 

Up to a few years ago, the association between the risk of suicidal ideation and behavior and the use of antiepileptic drugs had been explored in very few studies. In an observational study of 517 consecutive patients taking levetiracetam, four (0.7%) reported suicidal ideation [Id ]. The incidence of suicidal ideation and behavior resulting from antiepileptic drug exposure in clinical trials is unknown, because most published data group all psychiatric adverse events together rather than reporting suicidality by itself [15, 16, 17 ].
