Posaconazole Phenobarbital

Clinically important, potentially hazardous interactions with aspirin, carbamazepine, clarithromycin, enoxaparin, HIV protease inhibitors, itraconazole, ketoconazole, phenobarbital, phenytoin, posaconazole, ritonavir, St John s wort, voriconazole... 

Limited evidence su ests phenobarbital causes a marked decrease in itraconazole levels, and might decrease ketoconazole levels. Phenobarbital is also predicted to decrease posaconazole levels and markedly decrease voriconazole levels. 

Based on the evidence with phenytoin , (p.552), the manufacturer of posaconazole predicts that phenobarbital will reduce posaconazole levels, and therefore suggests avoiding the combination unless the benefits outweigh the risks. If concurrent use is necessary monitor for posaconazole...
