Anterior stromal infiltrates can develop under the dendritic HZO lesions and appear as hazy, granular, nummular, subepithelial opacities. They have been observed in close proximity to enlarged comeal nerves, which possibly represents a perineuritis from viral destruction of the sensory nerves.
