Acceptance of some level Compliance driven Focusing on Lost Time/ Recordable Not Focusing on Hazard  

Management Driven Safety Systems (Process), I want to do it.  

The major concern deals with the trust factor. Employees will question why this approach is being offered and may question the sincerity of the effort. 

A real pitfall in this phase deals with sustaining the effectiveness of the initiated safety activates. When an organization fails to maintain these efforts, the setback could be worse than doing as employees and management alike will be less inclined to be made a fool twice. Mistakes will be made and that is acceptable, if one openly acknowledges those mistakes and take the necessary steps to correct and prevent their recurrences. 

One major concern at this level is the sustainability of this new safety culture, especially if there are changes to requirements, management, union, owners, and the like. If the safety culture has not had a chance to cement itself, sustainable changes could damage the sustainability of this safety culture.
