Monthly Safety Activity Form

For those organizations that operate multiple warehouses, monthly safety performance can be tracked from a central location. Figure 2-3 presents a form for each warehouse to log their monthly activity on a calendar year basis. A copy of this form can be forwarded to the head office for the purpose of tracking compliance activities associated with the established safety program.

The TMI-2 incident occurred on March 28, 1979 on March 31, B W formed a Criticality Analysis Task Force composed of penoniiel from the Company s reactor physics unit and their nuclear criticality safety unit. The Task Force at maximum strength utiliaed ten individuals and completed all pertinent evaluations in two weeks advisory activity continued for several months.
