Interview the employees who perform the tasks that require this practice. 

Observe situations where employees perform this practice. 

Interview employees who have been involved in incidents. 

Read any policies and procedures, manufacturer s instructions, job safety and job hazard analyses, and other materials that might describe the practice. Talk to experts about the practice. 

If the steering committee decides on (i), identify all the potential antecedents and consequences for the practice. If it chooses (ii), identify the actual antecedents and consequences for the practice in that situation. The steering committee members use their experience and knowledge of the situation as well as the comments made on the observation checklists and the descriptions on the injury
