Methotrimeprazine levomepromazine

High efficacy opioid plus a sedative/anxiolytic (diazepcim) or a phenothiazine tranquUliser, e.g. chlorpromazine, levomepromazine (methotrimeprazine) (which also has analgesic effect). 

Phenothiazines do not produce analgesia but can potentiate the efficacy of other analgesic drugs, such as the opioids (p. 276). An exception to this is levomepromazine (methotrimeprazine), a phenothiazine used formerly in the horse in combination with the opioid etorphine (Petts Pleuvry 1983). Phenothiazines lower the MAC of inhalation anesthetics. In one controlled study, acepromazine was shown to decrease the MAC of halothane by approximately 37% (Doherty et al 1997b).

Tranquillizing drugs based on the phenothiazine nucleus potentiate the action of narcotic analgesics [157] although there are some reports to the contrary [158] and certain derivatives, notably methotrimeprazine (levomepromazine, Levo-prome, Veractil) (—)-(LV), are claimed to have intrinsic analgesic activity. In mice, (-)-(LV) is more potent than morphine in a variety of tests [159, 160] for example hot-plate EDso value of 1.02 mg/kg, that of morphine 2.1 mg/kg.

Compounds considered here include substituted phenothiazines, thioxanthenes and butyrophenones. Not all phenothiazines are antipsychotics, notably promethazine, an antihistamine, and methotrimeprazine (levomepromazine), which is used as a sedative in horses. However, their structural similarities mean that they behave in a similar way analytically. With molar extinction coefficients of ca 30,000 LmoF cm in the range 250-260 nm, phenothiazines can be measured using UV detection down to about 1 ng on-column. However, ED offers an increase in sensitivity of an order of magnitude or more. The ring sulfur appears to be the site of oxidation (0.8-1.0 V vs Ag/AgCl, Table 4.1) as evidenced by the fact that secondary and primary aliphatic amine, and even A-oxide metabolites, show similar responses to the parent compound. However, sulfoxide metabolites do not respond unless they are reduced at the first electrode of a two-electrode system (Figure 3.7).
