Fixed ladders

Manufaotured fixed iadders ANSi A14.3-1984 -Amerioan Nationai Standard for Ladders - Fixed -Safety Requirements. 

As stated in Section 11.4, if a manhole has been removed but entiy has not been authorized, the manhole should be covered by a fixed barrier, not just a plastic sheet. A ladder inside a manhole that is protected by only a loose cover is almost an invitation to enter.

The two first equations follow from the commutation relations of the angular momentum components, Jf F = X, Y, Z, along space fixed axes. The latter two are not as obvious as they may appear at first glance. Since Jx and Jy do not commute with h we cannot factorize the vector space to treat // and Jz in a separated basis of vectors, (In, />, such as it is usually assumed when discussing the two-dimensional harmonic oscillator. The incident concerns an item of plant temporarily fitted on the reactor building polar crane carriage with a view to dismantling the access gangway to the crane arch. The gangway is a structure fixed on one of the polar crane beams which it was decided to replace by a simple hooped access ladder in 1988. 

Circumstances of the accident One day in Autumn 1993 Rhys was participating in a gymnastics lesson when he fell from a ladder supported by a fixed and portable apparatus frame and broke his arm. At the time, Rhys was performing what is known as a low impact movement, which involved climbing the apparatus. The medical report indicates that when climbing the ladder Rhys s hands were somewhat slippery and he slipped off the climbing ladder. Rhys claims that he was at the top of the ladder and intended to climb over the top of the frame to the other side. Mr C claims that the general policy is that children do not climb over the top of the frame because of the wires. 

A standard railing is not required in places where there is an entrance to a ramp, stairway, or fixed ladder.
