On initial start-up and shut-down the heat exchanger can be subjected to damaging thermal shock, overpressure or hydraulic hammer. This can lead to leaky tube-to-tubesheet joints, damaged expansion joints or packing glands because of excessive axial thermal, expansion of the tubes or shell. Excessive shell side flowrates during the "shake down can cause tube vibrations and catastrophic failure. 

I. Leakage—joints and packing this factor accounts for the possibility of leakage from gaskets, pump and other shaft seals, and packed glands. The factor varies from 0.1 where there is the possibility of minor leaks, to 1.5 for processes that have sight glasses, bellows or other expansion joints. 

The apparatus consists essentially of a cast iron crucible and cover or fusion pot in which is placed a nickel crucible or dish. The complete apparatus is illustrated in Fig. II, 50, 1. The fusion pot A carries a combined thermometer well T and an inlet tube BCD, and a wide outlet tube E. The thermometer well T is closed at the bottom (welded) and projects 6-7 mm. below the wider inlet tube to which it is welded near the top.
