Aprepitant may be affected by paroxetine, CYP2C9 substrates (eg, phenytoin, tolbutamide, warfarin), CYP3A4 substrates (eg, alprazolam, cisapride, dexamethasone, docetaxel, etoposide, ifosfamide, imatinib, irinotecan, methylprednisolone, midazolam, paclitaxel, pimozide, triazolam, vinblastine, vincristine, vinorelbine), and oral contraceptives. [Pg.1007]

E Itraconazole. Imatinib is primarily metabolized by the CYP3A4 hepatic enzyme system. Drugs that may inhibit this enzyme (such as ketoconazole, itraconazole, erythromycin, clarithromycin, etc.) may impair clearance of imatinib and result in increased toxicity. Imatinib itself is also a fairly potent inhibitor of the CYP3A4 enzyme and may result in toxicity due to other drugs that are substrates for this enzyme (such as simvastatin, warfarin, benzodiazepines, etc.). Imatinib increases serum tacrolimus levels and is therefore predicted to increase ciclosporin levels. Imatinib may also increases the levels of some benzodiazepines (e.g. midazolam), some calcium-channel blockers (e.g. nifedipine), oestrogens, paracetamol, pimozide, and warfarin. In one case lansoprazole was suspected to have raised imatinib levels. [Pg.637]

The manufacturers say that because warfarin is metabolised by CYP2C9, patients needing anticoagulation should be given low-molecular-weight or standard heparin instead. This recommendation is based on an observation in one patient and in vitro studies that show that imatinib can inhibit CYP2C9.
