Hydroxyurea recurrent

We are also currently conducting a phase I/II trial in patients with unresectable, advanced, or locally recurrent HNC with potential benefits from reirradiation (Table 4). We chose to evaluate the promising radio sensitization effects of irinotecanby combining it with our FHX regimen (79). Patients were given five cycles of irinotecan (5-15 mg/m2, d 1-5), continuous infusion 5-FU (600 mg/m2, d 1-5), hydroxyurea (500 mg BID for 11 doses), and hyperfractionated twice-daily radiotherapy. Preliminary analysis of patients in the phase I setting established a MTD of irinotecan at 10 mg/m2.

Over the next several years, she had recurrent episodes of reactive airway disease. At the age of 4 years, she had a life-threatening episode of acute chest syndrome requiring admission to the intensive care unit and exchange transfusion. She was subsequently transfused with red blood cells monthly for 6 months to prevent recurrence. Two years later, she was again admitted to the intensive care unit with acute chest syndrome. During this admission, she was found to have Streptococcus pneumoniae sepsis and pneumonia. She again received RBC transfusions monthly for 6 months. Following this course of transfusion therapy, she was offered therapy with hydroxyurea, but this therapy was never instituted. 

Modalities to prevent priapism are limited and not well studied. Pseudoephedrine (30 or 60 mg/day given orally at bedtime) and leuprolide, a gonadotropin-releasing hormone, have been used to decrease the number of recurrent episodes of priapism. Hydroxyurea therapy may also be useful. Recently, low-doses of an antiandrogen, bicalutamide, have been used in two patients with SCD and one patient with spinal cord injury for treatment of recurrent and refractory priapism without major side effects. Hydroxyurea, an antimetabolite with antineoplastic properties (60 to 80 mg/kg p.o. for a minimum of six weeks), is used in the treatment of melanoma chronic myelocytic leukemia recurrent, metastatic, or inoperable ovarian cancer squamous cell carcinoma of the head and neck polycythemia vera and essential thrombocytosis (see also Figure 15).
