During a polymer plant shutdown, the operator was supposed to remove the plug from and connect a nitrogen purging hose to a fitting immediately downstream from an isolating block valve connected to a depressurized system. Instead, the employee mistakenly connected to a pressurized fitting immediately upstream from the block valve, into a 350-psi [Pg.157]

This company s approach was somewhat different than the approach described in the previous incident. The polymer plant implemented a procedure to require force-loaded check valves for backflow prevention on any temporary connection to piping that contained flammables.
