In another cocaine body packer, non-surgical management was followed by the development of a giant gastric ulcer (377). 

Although these findings may apply to an experimental cortisone ulcer in animals, Hitzelberg in our laboratory obtained less definite results in man (H22, H23). Fifteen arthritic patients received 1 g prednisolone in 6 weeks. In the course of treatment, there was never an indication of peptic ulcer formation, although the output and concentration of pepsin in the basal secretion increased on the average about 2-fold. The right lobe of liver, especially the dorsal segments, is most frequently involved (60-70%). In 20-30% of cases, both lobes are affected. Solitary abscesses are localized in the right lobe of liver in 80-90% of cases. Multiple abscesses can be observed in approx. 40% of patients. Nineteenth-century physiologists asked whether gastric secretion follows hyperemia, whether hyperemia follows gastric secretion, or whether the two are independent phenomena. In the middle of the century Friedrich Giinzburg said that gastric ulcers result from excessive vagally stimulated acid secretion, and Rudolf Virchow said, on the contrary, that they are caused by reduction or complete elimination of blood flow to the mucosa at the site of ulceration. Neither Giinsburg nor Virchow had any experimental evidence for his belief, and clinicians debated the alternatives for more than 100 years.
