The hterature describing randomized double-blind studies of effectiveness of Fiorinal and Fioricet is very limited. In one study Fiorinal PA, aspirin 200 mg and acetaminophen 200 mg (replacing the phenacetin in the old formulation) with caffeine 40 mg and butalbital 50 mg were compared to placebo. Among patients, 84% responded to treatment, compared to placebo response of 27% (P < 0.05) [15,16]. In another study, Fioricet (acetaminophen 325 mg, caffeine 40 mg, and butalbital 50mg) was statistically more effective when compared to placebo [17]. 

Tablets 50 mgbutalbifal, 40 mg caffeine, and 325 mg acefaminophen (Arcet, Dolmar, Esgic, Fioricet, Isocet, Pharmagesic, Repan) 50 mg butalbital, 40 mg caffeine, and 500 mg acetaminophen (Esgic-Plus, Zebutal). 

Rx with acetaminophen (Tylenol No. 2, Tylenol No. 3, Tylenol No. 4) with aspirin (Empirin No. 3, Empirin No. 4) with chlorpheniramine (Codeprex) with guaifenesin (Robitussin AC) with APAP (Capital, Aceta) with APAP, butalbital, caffeine (Fioricet, Phenaphen) with aspirin (Fiorinal)... 

The barbiturate butalbital is sold in combination with aspirin or acetaminophen (Fiorinal or Fioricet , respectively) for treatment of migraine and chronic tension headaches. When providing a urine sample, consumption of either of these drugs must be mentioned.

Fiorinal and Fioricet are known as combination analgesics that were introduced to the market as medications for abortive treatment of headache. Fiorinal was formulated in 1950s and contained phenacetin as well as aspirin, caffeine, and butalbital. Later, the phenacetin was withdrawn from the market in the 1960s and compound Fiorinal thereafter consisted of aspirin 325 mg, caffeine 40 mg and butalbital 50 mg. At the time of reformulation, Fioricet was introduced to the market, substituting acetaminophen 325 mg for aspirin [8]. 

Combination analgesics represent important alternatives for those patients who cannot or should not take vasoconstricting medications or opioids. The acetaminophen compound of Fioricet is also a major alternative to those patients who cannot use NSAIDs. Nevertheless, Fioricet and Fiorinal contain the shortacting barbitmate butalbital and may be habit-forming and addictive.
