Fiber deficiency disorders

Current interest in dietary fiber began about ten years ago when Burkitt and Trowell hypothesized that diabetes and heart disease were fiber deficiency disorders (1 ). Since then there has been increasing research on the effects of dietary fiber on carbohydrate and lipid metabolism. Such work was pioneered and stimulated in large part by Dr. J. Anderson of Lexington, Kentucky who first used high carbohydrate/high fiber diets to treat diabetes 2) and Dr. D. Kritchevsky who has examined the effects of fiber on the metabolism of cholesterol O) ...

Calcium absorption is reduced by high pH complex-ing agents such as oxalate, phytate, free fatty acids, and phosphate and shortened transit times. These factors are probably of clinical importance only when associated with vitamin D deficiency, marginal calcium intake, or malabsorption disorders. Absorption is also reduced by increased intake of protein, fat, and plant fiber increasing age stress chronic alcoholism immobilization (e.g., prolonged hospitalization) and drugs such as tetracycline, thyroid extract, diuretics, and aluminum-containing antacids. 

Urine is preferred for these tests. The small volumes required make it practicable to spot the urine onto cotton fiber filter paper and allow it to dry out, for ease of shipment by mail. The limited experience in applying MS/ MS for these analytes suggests that the levels of pathognomonic metabolites should significantly exceed control values for purines and pyrimidines regardless of the patient s clinical state (Table E.4).
