Dosing for pediatric patients in the 50-70 kg range can be more closely titrated by using a ratio to determine dose/kg 1/70 x adult dose = dose/1 kg. 

The usual pediatric dosage range is 2 to 4 mg/kg/day in 2 equally divided doses (eg, 1 to 2 mg/kg twice daily). Dosage calculated by weight generally produces plasma levels in a therapeutic range similar to that in adults. Do not use doses more than 16 mg/kg/day. 

Pediatric - Initial dose should not exceed 750 mg/day. The usual maintenance dose in children ranges from 500 mg to 1 g/day, although occasionally 2 g or rarely 3 g/day may be necessary. 

Pediatric patients 4 to 17 years of age with JRA were administered 0.4 mg/kg of etanercept twice weekly for up to 18 weeks. The mean serum concentration after repeated subcutaneous dosing was 2.1 mcg/mL (range, 0.7 to 4.3 mcg/mL).

The dose shown is during the first week of life. The daily dose should be increased by approximately 33-50% after the first week of life. The lower dosage range should be used for neonates weighing less than 2 kg. After the first month of life, pediatric doses may be used. 

Compliance may be more difficult to achieve in pediatric practice than otherwise, since it involves not only the parent s conscientious effort to follow directions but also such practical matters as measuring errors, spilling, and spitting out. For example, the measured volume of teaspoons" ranges from 2.5 to 7.8 ml. The parents should obtain a calibrated medicine spoon or syringe from the pharmacy. These devices improve the accuracy of dose measurements and simplify administration of drugs to children.
