Communication inadequacy

There were several dysfunctional interactions and communication inadequacies among the correctly operating aircraft equipment. The most obvious unsafe interaction was the release of two missiles in the direction of two friendly aircraft, but there were also four obstacles to the type of fighter-helicopter communications that might have prevented that release.

The other physical process being controlled here, public health, must be examined in the same way. There were very few controls over public health instituted in Oakbridge, the community surrounding the plant, and the ones that did exist were inadequate. The public had no training in what to do in case of an emergency, the emergency response system was woefully inadequate, and unsafe development was allowed, such as the creation of a children s park right outside the walls of the plant. The reasons for these inadequacies, as well as the inadequacies of the controls on the physical plant process, are considered in the next section. 

Modem readers can hardly imagine the absolute novelty of those findings and the embarrassing inadequacy of vast sectors of the chemical community to assess or even understand them. As a matter of fact, until 1954 it was largely believed that stereoregular polymers can only have natural origin (as can be read in the motivation of the Nobel Prize to Natta [25]). That the world was just not ready is demonstrated... 

In Malaysia reports indicate that clinical vitamin A deficiency is uncommon, apart from severe PEM admissions to pediatric wards (Y. H. Chong, 1981, personal communication). There is evidence of subclinical hypovitaminosis A and dietary inadequacy, especially among Malay children from traditional families (Chen, 1972). One study reported one-third of the preschool children and one-fifth of the school children on a rubber plantation had serum vitamin A levels >20 xg/dl (Ng and Chong, 1977). Mean vitamin levels of breast milk are low (13 juig/dl mature milk) among rural women, but they are adequate among urban mothers (63 xg/dl) (Y. H. Chong, 1981, personal communication).
