Collaborative care definition

Collaborative care has many definitions. It does not refer to split care, which usually implies that the physician treats the biological part of the patient by prescribing medication, and the therapist does the rest. Although split care may be attractive in terms of cost and ease for the provider, it is inadequate care. Patients need their therapist and physician to communicate, particularly in the creation and maintenance of a treatment plan. 

The framework in Figure 9.18 illustrates a relationship between a Trader who makes Orders from a Distributor. In the framework, we don t care how the Trader gets rid of stock, nor how the Distributor acquires it. We have shown this as a degenerate collaboration, as it will next be refined as a unit. (Notice that we ve made all the types substitutable except Date. So we would likely have Date defined as an actual type somewhere in the package in which this framework definition appears or in the packages it imports.)... 

By careful control of the conditions of the reaction one can obtain preferentially oxidation of sulfur to its four coordinate oxidation state and by using a second set of conditions one can obtain the oxidation to the six-coordinate sulfur species. The dynamic NMR study of SF3CF2SF3 is currently in progress in collaboration with A. H. Cowley (60). This differentiation of oxidation states is extremely promising, and work in progress shows that this is not at all an isolated situation. Mercaptans and other organosulfur compounds definitely exhibit this capacity in fluorine reactions. 

The ability to provide pharmaceutical care is the profession s ultimate goal. In this practice, the pharmacist shares responsibility and accountability for the patient and drug therapy outcomes. The principal goal of pharmaceutical care is to achieve definite outcomes from medication use that improve the patient s quality of life. These outcomes include cure of a disease, elimination or reduction of disease symptoms, slowing a disease process, prevention of disease, and/or diagnosis of disease. Strong communication skills are required to establish a collaborative relationship with the patient, prescriber, and/or other members of the health care team.
