Thought content. Patterns in a child s thinking may include suicidality, homicidality, paranoia, delusions, preoccupations, anxieties, and themes emerging from a disorganized thinking pattern. 

The three nurses involved in this case were indicted for criminally negligent homicide in the death of the baby. ISMP provided a systems analysis and expert testimony at trial and identified over 50 different failures in the system that allowed this error to occur, go undetected, and ultimately, reach a healthy newborn child, causing his death. Had even just one of these failures not occurred, either the accident would not have happened, or the error would have been detected and corrected before reaching the infant.

Peck (1987) described a patient who was regularly beaten by his father as a child. In 488 children and adolescents (aged 7-17 years) with an anxiety disorder who were randomized to 12 weeks of sertraline up to 200 mg/day, cognitive behavioral therapy, the combination of these, or placebo, suicidal and homicidal ideation were no more common with sertraline than placebo no child attempted suicide [9 ].
