Blank signature

Most of the information required In Part I and all of the Information required In Part II of Form R can be filled In and photocopied and attached to each chemical-specific report. Part I must have an original signature on the certification statement and the trade secret designation must be entered as appropriate. You have the option to complete Part II for only the off-site locations that apply to the Individual chemical cited In the report g you can list all off-site locations that apply to all chemicals being reported and Include a photostatic copy of this Part II with each individual report. Part III must be completed separately for each chemical. Part IV provides additional space, if needed, to complete the information required by the preceding sections of the form. Include Part IV In your report, even it it Is blank. 

Production records must be completed in ink, preferably black or blue ink. Records should be completed in conjunction with the completion of each step. Records should never be completed before the task has been performed nor should tasks be completed with the entries made to the record at a later time. A space for an entry on a record should never be left blank. All notations expected on the production record must be either completed or the record should indicate why no entry was necessary. Completed production records should be reviewed by a supervisor to ensure that the record is complete with no blank spaces and that appropriate entries have been made. The supervisor acknowledges his/her review by signature or initials and date. 

The clinical trial monitor is a temporary member of the site team. A good monitor will conduct scheduled visits, and the investigator and the site staff should provide sufficient time to answer questions and correct data in the CRF that has been transferred incorrectly from source documents. Often negative answers are left blank rather than answered and signatures are... 

The same sample is again heated to the original temperature for the same holding time, and the measurement procedure is repeated. For inclusion-free samples, this re-extracf step will yield He at levels indistinguishable from that of the preceding hot blank. However, a substantial amount of He in this step has been found to be the telltale signature of mineral inclusions, at least in the case of apatite. If such He is found, the analysis is rejected as likely to be compromised by inclusions. 

Prescriptions for controlled substances should be dated and signed on the day of their issuance and must bear the full name and address of the patient and the printed name, address, and DEA number of the practitioner they should be signed as one would sign a legal document. Preprinted orders are not allowed in most states, and presigned blanks are prohibited by federal law. When oral orders are not permitted (schedule 11), the prescription must be written with ink or typewritten. The order may be prepared by a member of the physician s staff, but the prescriber is responsible for the signature and any errors that the order may contain. 

It is unclear how the volunteer signified consent. Note that this form does not contain a blank for the volunteer s signature. 

At the end of the experiment, the page at the end would be stamped, witnessed with full signature, and dated by the same witness or witnesses who had initialed the daily entries. Any remaining unused space reserved for the experiment would be crossed out in the manner prescribed under Blanks," above.
