Benemid

In those with gout, the serum uric acid level is usually elevated. Sulfinpyrazone increases the excretion of uric acid by the kidneys, which lowers serum uric acid levels and consequently retards the deposit of urate crystals in the joints. Probenecid (Benemid) works in the same manner and may be given alone or with colchicine as combination therapy when there are frequent, recurrent attacks of gout. Probenecid also has been used to prolong the plasma levels of the penicillins and cephalosporins.

Penicillins and Probenecid. A number of organic acids are actively transported from the blood into the tubular urine and vice versa. In some situations, these agents interfere with each other s excretion. Probenecid (e.g., Benemid) can increase the serum concentrations and increase and prolong the activity of penicillin derivatives by blocking their tubular secretion. This is an interaction that has been used to therapeutic advantage in the treatment of certain infections.

Understand that the healthcare provider might prescribe that probenecid (Benemid) and cephalosporin are to be taken together. The goal is to extend the half-life of cephalosporin without having to increase the dose. 

This figure refiects the protein-binding of BSP (see Section 4.2). There is evidence that less than 2% of the BSP in plasma after a standard dose is unbound (G16). Allowance for this gives a minimal clearance for BSP of 250 ml/min, which suggests that either free BSP or its conjugate or both are secreted by the renal tubules. Benemid, which is known to depress renal tubular activity (M6), reduces the renal excretion of BSP (B28) which supports this view.

In support of this suggestion, it is known that high phosphate feeding may produce tetany and that low phosphate feeding or Benemid administration may relieve it in hypoparathyroidism (P2). Conversely, Albright (A2) has demonstrated that in primary hyperparathyroidism...
