Bambuterol has been reported to alter the metabolism of mivacurium (17). Bambuterol has a dose-dependent inhibitory effect on plasma cholinesterase activity and prolongs the effects of suxamethonium. Bambuterol 10 mg was given to 28 patients 2 hours before an elective operation requiring general anesthesia. The patients given bambuterol had a 67-97% fall in plasma cholinesterase activity, leading to reduced clearance of mivacurium. This resulted in a shorter onset and a 3- to 4-fold prolongation of action of the neuromuscular blockade produced by standard doses of mivacurium. 

Bambuterol, a beta2-adrenoceptor agonist that is used to relieve bronchospasm, approximately doubles the duration of action of suxamethonium (SEDA-14, 114) (279). Plasma cholinesterase activity was reduced significantly even 10-12.5 hours after a single dose of 30 mg had been given to patients. The interaction is due to the binding of carbamate groups to plasma cholinesterase.

Bambuterol is an inactive prodrug which is slowly converted enzymatically in the body to its active form, terbutaline. The carbamate groups that are split off can selectively inhibit the plasma cholinesterase that is necessary for the metabolism of suxamethonium and mivacurium.
