Clinically important, potentially hazardous interactions with albuterol, alpha-blockers, amitriptyline, amoxapine, atenolol, beta-blockers, carteolol, chlorpromazine, clomipramine, cocaine, desipramine, doxepin, ephedra, ergotamine, furazolidone, halothane, imipramine, insulin detemir, MAO inhibitors, metoprolol, nadolol, nortriptyline, oxprenolol, penbutolol, phenelzine, phenoxybenzamine, phenylephrine, pindolol, prazosin, propranolol, protriptyline, sympathomimetics, terbutaline, thioridazine, timolol, tranylcypromine, tricyclic antidepressants, trimipramine, vasopressors...

Therefore, because of its pronounced anticholinergic and potential arrhythmogenic effects, amitriptyline is contraindicated in the acute recovery phase of myocardial infarction, congestive heart failure, angina, prostatic hypertrophy, paralytic ileus, and urinary retention, and in patients undergoing surgery with general anesthetics that may cause arrhythmias (e.g., halothane) (see Table 16).

Tricyclic antidepressants may increase the risk of arrhythmias and hypotension during anaesthesia. Tachyarrhythmias have been seen in patients taking imipramine who were given halothane and pancuronium. Some very limited evidence su ests that amitriptyline may increase the likelihood of enflurane-induced seizure activity. A man taking maprotiline and lithium developed a tonic-clonic seizure when given propofol. Tricyclics may cause an increase in the duration of barbiturate anaesthesia and lower doses of barbiturates may be required.
