The development of a formulary system within an organization rests with a multidisciplinary committee. In the hospital and health system setting, this is typically called the P T committee. Virtually all hospitals and health-systems have a P T committee.P T committees usually meet six to eight times annually. An ASHP Position Statement on formulary management declares that decisions should be based on clinical, quality of life, and pharmacoeconomic factors that result in optimal patient care. - It advises against decisions solely based on economic factors. The Position Statement also recommends that decisions must include active and direct involvement of physicians, pharmacists, and other appropriate health care providers. This may include dieticians, nurses, administrators and quality management coordinators. [Pg.363]

You will get broader interest and involvement in an incident analysis process if corrective action plans are applied to all relevant work areas. This promotes a systems perspective rather than the piecemeal "band-aid" approach common to so many work cultures. Look at the bigger picture. Use the results of an incident analysis to improve relevant environment behavior and person factors plantwide. Some drugs situations are frankly confusing and need to be explored in greater depth. For example, tumor necrosis factor (TNF) is produced in the body as a natural defense against the many spontaneous tumors that arise in the body and has been suggested as a treatment for cancer, which may very well be effective under some conditions. However, TNF is also believed to be involved in the pain associated with rheumatoid arthritis, and monoclonal antibodies with specific activity against TNF are now on the market. The use of TNF is therefore a complex issue and needs to be evaluated with considerable care. [Pg.3]

Rates of Gas-Phase Reactions. Reaction rates have been reported for only a few CVD gas-phase reactions, and most reports are primarily for the silane system. Because of the high temperatures and low pressures used in CVD, the direct use of reported gas-phase rate constants must be done with care. In addition to mass-transfer and wall effects, process pressure may be another factor affecting reaction rates. Process pressure affects major CVD processes, such as the deposition of Si from SiH4 and GaAs from Ga(CH3)3, reactions that involve unimolecular decomposition. The collisional activation, deactivation, and decomposition underlying these reactions can be summarized qualitatively by the following reactions (139, 140) ... [Pg.229]

There is no method of making a plant truly inherently safe, since there is always risk when human activity is involved. But, if we carefully examine the technology available to us, we can make chemical plants inherently safer than they might be without such an examination. We can determine that a plant can be safe, but there are many factors that will determine whether a plant will be safe. CEFIC, the European Council of Chemical Manufacturers Federations (CEFIC, 1986), reports these include human factors that are so difficult to quantify that they are rarely taken into consideration. They include the human side of plant management, operation, and maintenance. Designers cannot do much about these human factors, but they can often do a lot to make the plant easy to operate, and reduce the chances of accidents that may result from human error and mechanical failure. [Pg.74]
