Anomalies of the ICA are infrequent, and aplasia of the ICA is a rarity (Van de Perre et al. 2004). In hypoplasia, the vessel tapers off behind a normal proximal segment and can sometimes be followed up to the circle of Willis. Occasionally differential diagnosis is difficult against an arterial dissection or a very high grade stenosis with a collapsing distal lumen (pseudoocclusion). Many investigators identified smoking as a strong risk factor for atherosclerotic renal artery stenosis Hadj-Abdelkader [21] found that 70-80% of patients with this diagnosis were smokers and frequently these stenoses are bilateral [22] and frequently the source of cholesterol microembolism [23]. 

Tc-DTPA is specific for the diagnosis of renal artery stenosis (Gruenewald and Collins 1983). Angiotensin-converting enzymes (ACE) inhibitors such as captopril reduce filtration pressure and thus cause a fall in GFR, as manifested by decreased renal uptake of Tc-DTPA. In patients who do not have renovascular hypertension, the GFR of each kidney is unchanged.
