Severe incident

As already stated, it is usual to test for the presence of flammable gas or vapor with a combustible gas detector before maintenance, especially welding or other hot work, starts. Several incidents have occurred because tests were carried out several hours beforehand and conditions changed. 

One incident is described in Section 2.8 (a). Several incidents have occurred because drums or bottles were unlabeled and people assumed that they contained the material usually handled at the plant. In one case, six drums of hypo (sodium hypochlorite) had to be added to a tank of water. Some of the drums were not labeled. One, which contained sulfuric acid, was added after some of the genuine hypo and chlorine was given off. The men adding the material in the drums were affected by the fumes. 

Several incidents have occurred because similar names were confused. The famous case involving Nutrimaster (a food additive for animals) and Firemaster (a fire retardant) is w ell known. The two materials were supplied in similar bags. A bag of Firemaster, delivered instead of Nutrimaster, was mixed into animal feeding stuffs, causing an epidemic of illness among the fami animals. Farmers and their families were also affected [3]. 

Several incidents in which moisture has contributed to fires or explosions, in some of which water was definitely the sole oxidant, in zirconium, magnesium, uranium and thorium scraps or powders are retailed, largely sourced from an earlier paper. However, a plutonium fire was extinguished with water. 

There are several incidents in the text where glass has been involved as a direct cause, either because of its chemical properties (alkalinity, corrosive attack), or its physical properties (actinic transparency, or mechanical limitations under stress). Worst credible incident—The most severe incident, considering only incident outcomes and their consequences, of all identified incidents and their outcomes, that is considered plausible or reasonably believable. 

This is not a particularly supportive department for anyone. The chairman, in particular, is not very supportive. In fact, I have had several incidents where he has been just bitingly critical of my teaching. I thought he was excessive. He has also made some racially insensitive remarks to me over the years.

Adult rats were injected with MOG, and then monitored daily to register the parameters, including EAE severity, incidence, cumulative score, and mortality. The study involved groups of 12 animals each. 

Aluminium dissolves readily, and exothermically, in aqueous alkali, generating highly combustible hydrogen and hydrides of other non-metals and metalloids which may be present. This has given rise to several incidents below. 

Because of the aforementioned circumstances, the loss of control of the phenol-formaldehyde reaction has been the cause of a number of severe incidents in chemical batch reactors during the last decades [12], These incidents have caused many injuries and, in the worst case, even fatalities among the plant operators. Other severe consequences have been the evacuation of residents in the surrounding area due to chemical contamination and a protracted stop in the plant production. 

Residential/Environmental Exposures. Several incidents in which populations were exposed to potentially high levels of 2,3,7,8-TCDD include an industrial accident that occurred during the production of 2,4,5-TCP at the ICMESA plant in Seveso, Italy and the spraying of roads and other places...
