Sample Accident Investigation Report

Contributing causes (check each appropriate causal factor) [Pg.425]

All the information of the accident sample was from the internet, in which the information of laws and regulations, policy files, and investigation reports were all from official websites, and the information of reports related to the accidents were all from Xinhua Net, People s Net, CCTV and other mainstream media. [Pg.686]

Trend analysis can be confused or invalidated by a sample that is too small. If the charting or analysis is limited only to major incidents, there will often be too few within a period to arrive at meaningful conclusions. For example, a facility with one thousand employees may experience only one or two serious incidents per year, and several years worth of data would be needed to make any meaningful statistical analysis. Minor incidents and near misses can be as useful in trend analysis and preventive prediction as major incidents. All process incidents should be reported, classified, and investigated as appropriate. The severity of an incident is frequently more a function of chance than actual fundamental system differences among accidents and near misses. [Pg.281]

Preservation of Evidence. All equipment and supplies, including but not limited to unused medications, sheets, towels, syringes, samples from the lab, nn-used blood prodncts, and equipment, should be retained after a medical accident. These items can be used in the investigation to ensirre a safe environment. Medical device maliunctions should be tagged per the Medical Device and Product Reporting, Alerts, and Recall Policy, 900.00. [Pg.289]
