2 Safety Management System Review, Not Accident Investigation 

The company quickly looked internally and found a few things that shocked them  

Upon review, it turned out that there had been other, apparently unrelated accidents at other factories around the world. 

But the factory audit records said that these factories were in safety program compliance and met local laws and standards. 

many of the safety programs were not fully implemented, but it was unclear why.
