When the condition of 17a-hydroxylase deficiency occurs, the production of cortisol and sex steroids (except for progesterone) is impaired and the production of miner-alocorticoids increased (see Figure 51-4, Chapter 51). With tliis deficiency, plasma concentrations of corticosterone, 11 - deoxycorticosterone, progesterone, and pregnenolone are elevated. Boys exhibit pseudohermaphroditism and deficiency in development of secondary sex characteristics.

Representative values for the concentrations in plasma of the major steroids under review here are given in Table 1. These values bridge what is at present an important boimdary between relatively easy and rather diflScult problems of sensitivity with current techniques. In rats provided with suma extract (5 g suma/100 ml water, estimated dose per animal not reported) as the sole source of drinking water for 30 days, increases in plasma concentrations of estradiol-17p and progesterone in female mice, and of testosterone in male mice, were observed (Oshima and Gu 2003). 

In 34 HTV positive women no clinically significant interaction was noted when NVP was coadministered with the combined oral contraceptive (0.15 mg desogestrel/0.03mg ethinyl oestradiol), however, co-administration of EFV resulted in high levels of progesterone including 3/16 subjects with levels which could indicate ovulation. The median plasma concentration of EFV 12 h post-administration decreased significantly by 22% [262 ]. 

In men, approximately 8 mg of testosterone is produced daily. About 95% is produced by the Leydig cells and only 5% by the adrenals. The testis also secretes small amounts of another potent androgen, dihydrotestosterone, as well as androstenedione and dehydroepiandrosterone, which are weak androgens. Pregnenolone and progesterone and their 17-hydroxylated derivatives are also released in small amounts. Plasma levels of testosterone in males are about 0.6 mcg/dL after puberty and appear to decline after age 50. Testosterone is also present in the plasma of women in concentrations of approximately 0.03 mcg/dL and is derived in approximately equal parts from the ovaries and adrenals and by the peripheral conversion of other hormones.
