Plasma proteins aspects

As already noted in Fig. 5.1, the ring opening of /3-lactam antibiotics is not only a major event in their interaction with bacterial enzymes, but occurs also in the mammalian body. Chemical hydrolysis in body fluids, metabolism by mammalian enzymes and interactions with plasma proteins, all influence the pharmacological activity of these antibiotics. These aspects will be considered below. We begin with the discussion of the chemical reactivity of the /3-lactam nucleus, which plays a key role in these events. 

Stress is frequently a trigger factor for depression in vulnerable patients. There is clinical evidence to show that CRT is elevated in the cerebrospinal fluid of untreated depressed patients, which presumably leads to the hypercortisolaemia that usually accompanies the condition. One of the consequences of elevated plasma glucocorticoids is a suppression of some aspects of cellular immunity. It is now established that many cellular (for example, natural killer cell activity, T-cell replication) and non-cellular (for example, raised acute phase proteins) aspects are abnormal in the untreated depressed patient. Such observations could help to explain the susceptibility of depressed patients to physical ill health.

One of the most important practical aspects of protein binding is the fact that drugs of similar or markedly dissimilar structure may compete for the same binding sites on the protein (7). Hence, multidrug therapy may lead to alteration in the plasma concentration and the rate of elimination of drugs, because of multiple competition for the plasma protein-binding sites.

More research has been devoted to the study of increased vascular permeability than to any other aspect of inflammation, and several excellent reviews on the subject have recently appeared . Increased vascular permeability here refers to the increased tendency for fluid and plasma proteins to pass through the vessel wall into the extravascular space. Various methods of studying the process have been described, usually involving either the assay of extravasated protein or protein-bound dyes such as trypan blue, or the measurement of the degree of swelling produced in a rat s paw after the subcutaneous injection of an irritant (see page 67).

As we expand our observations from the behavior of a few proteins to that of plasma, we may begin to feel justified in asking why the system performs rather than how it does. The question "Why do plasma proteins interact at interfaces " can then be interpreted to mean "what aspects of the behavior and interactions among purified plasma proteins can be seen as well in their natural habitat - the plasma -, and can these aspects be explained as being beneficial to our survival " What thus far had appeared as senselessly complex behavior of purified proteins at interfaces may become more reasonable in the context of many plasma proteins interacting at the mottled surfaces of cells in a way that will allow the survival of their host. Meanwhile, we will discover that purified proteins behave unlike their sibblings in vivo and that in the eyes of our plasma a purified protein adsorbed out of an artificial solution will not look like a film of the same protein deposited by the plasma itself.

Following are some aspects that limit the broad application of the depletion kits. The potential issue of employing this albumin depletion strategy is the limited sample size, typically less than 50 pL, which can be treated by the depletion kits. This sample size prevents the use of a large sample volume to obtain assay sensitivity enhancement. The depletion kits are relatively expensive. Perhaps the most significant drawback of the immunodepletion methods is nonspecific removal of plasma proteins of interest that may be bound to the removed proteins (Anderson and Hunter, 2006 Liu et al., 2006).
