Patient Group Directions information requirements

Once dispensed, the item is given to the patient or patient s representative who may pay the normal prescription levy (although this may be abolished in Scotland in time see Section 2.4.3) or if claiming exemption the patient or the patient s representative will be required to indicate the reason for exemption and sign the CP(US) form. Patients must also be advised of the normal procedure to obtain additional prescriptions and informed that they will not be able to receive a second successive supply of the medicine provided under this Patient Group Direction. The CP(US) form will then be endorsed and stamped as required.
