Acknowledgement. The authors would like to thank the Dow Chemical Company for its continuing support and encouragement in the development of the benzocydobutene technology. Many of the people cited in the references have contributed significantly to the results reported in this review. K.J.B. would like to thank NASA-Langley for their support in the development of much of the AB... [Pg.63]

Id meet to qualify and assure a reliable part. This assignment, undertaken by NASA MSFC in 1974, led to the first document for the procurement of adhesives to be used in hybrid microcircuits. After further coordination and revisions, this initial document was formally released as a specification in 1978 and revised in 1982 as NASA MSFC-SPEC-592, Specification For The Selection and Use of Organic Adhesives in Hybrid Microcircuits." For the first time, the critical requirements for ionic contaminants, electrical resistivity, die-shear strength, corrosivity, and outgassing were specified. Work on the development and evaluation of adhesives continued into the mid-1980s and, as a result, several commercially available adhesives were qualified." ... [Pg.30]

Is the risk acceptable When I asked a group of safety practitioners if the risk of 1 failure resulting in Loss of Crew (LOC) in 10,000 missions was acceptable, their overwhelming response was no. But, they could not reach a consensus to recommend the complete shutdown of space ventures. Those taking part in journeys to outer space are aware of the risks and volunteer for the opportunity to be astronauts, which is a form of employment. So, the managers at NASA and the Congressional Committees that provide oversight to NASA are also aware of the risks. And space ventures will continue. [Pg.279]

The CAIB report states that It was a small and logical next step for the discovery of foam debris damage to the tiles to be viewed by NASA as part of an already existing maintenance problem, an assessment based on experience, not on a thorough hazard analysis (CAIB, 2003 196). This statement is incorrect. As a result of the out-of-family anomaly for STS-87, a hazard analysis was conducted using fault tree methodology. A series of corrective actions were undertaken that, in the SSP s assessments, restored the tile damage to in-family levels. Experiments continued. [Pg.114]
