Migraine attack treatment

Sumatriptan is a 5HT agonist, which is indicated in the treatment of acute migraine attacks. Sumatriptan may cause vasoconstriction as a side-effect and therefore its use is reserved for patients in whom other conventional migraine analgesics have failed. 

Pethidine and morphine, used as opioid analgesics, may cause dependence following repeated administration. Zolmitriptan, a 5HT, agonist used in the treatment of acute migraine attacks, is not associated with dependence.

Clinical use Ibuprofen (Busson, 1986) is a nonsteroidal anti-inflammatory drug, commonly used for the treatment of mild to moderate pain. It is used in conditions like rheumatoid arthritis, osteoarthritis, joint and soft tissue pain, dental pain, postoperative pain, dysmenorrhoea and headache, including acute migraine attacks. 

Severe migraine attacks should be treated with a triptan, e.g. sumatriptan (below). In contrast to symptomatic treatments, triptans are best used during the established headache phase of the acute attack. Headache may return in 6-36 h in about one-third of patients, necessitating a second dose.

Frovatriptan is a serotonin 5-HT,-receptor agonist, which selectively agonizes 5-hydroxy-tryptamine, (5-HT,b/id) receptor, inhibiting excessive dilation of extracerebral and intracranial arteries in migraine. It is indicated in acute treatment of migraine attacks with or without aura m adults. 

Naratriptan is a serotonin S-HTj-receptor agonist that binds to serotonin (5-HT) Ig and Id receptors in intracranial arteries leading to vasoconstriction and subsequent relief of migraine headache. It is indicated in the treatment of acute migraine attacks with or without aura.
