1 Methods for Reducing or Preventing Human Error-Related Anesthetic Mishaps 

Many methods have been developed to reduce or prevent the occurrence of human error-related anesthetic mishaps. Two of these methods are presented below [2], 

This method for reducing or preventing human error-related anesthetic mishaps calls for appropriate actions in the following five areas [11]  

Step 2 is concerned with collating information and collecting subsets for sfudies and classifications. This calls for establishing a project team that has adequate access to the appropriate information. Step 3 is concerned with the grouping of problems, errors, and contributory factors under appropriate classifications.

This method for reducing or preventing human error-related anesthetic mishaps is composed of six sfeps, as shown in Figure 8.1 [12]. Step 1 is concerned with determining the existing state of affairs by reviewing items such as hazard alerts, medical defense reports, incident monitoring studies, mortality committee reports, observation studies, morbidity committee reports, and simulation studies.
