Methisazone

Methisazone (Fig. 5.2IB) inhibits DNA viruses (particularly vaccinia and variola) but not RNA viruses, and has been used in the prophylaxis of smallpox. It is now little used, especially as, according to the World Health Organization, smallpox has now been eradicated. 

The structure/activity relationships for the methisazone, 3a, derivatives against adenoviruses and poxviruses have been shown to be similar [78]. Pearson and Zimmerman [79] demonstrated that all three types of polioviruses are inhibited by 2-acetylpyridine JV-dibutylthiosemicarbazone, which is similar to 3a, by blocking viral RNA synthesis. A 3-substituted triazinoindole derivative of isatin was effective against several strains of rhinovirus in tissue culture the mechanism of action is unknown [80].

Among biologically active compounds - derivatives of isatin - there is methisazone (1-methylisatin 3-thiosemicarbazone), which has been used as a prophylactic agent against the smallpox virus [139-141], Methods for the synthesis of compounds with the general formula 146 have been developed in the search for new antiviral and antimicrobial agents [142] ... 

The reaction of 1-methylisatin and semicarbazone yielded methisazone, a compound that found use in the treatment of variola, a viral disease that has now been eradicated (Scheme 55). 

Clindamycin, dazomet, dimercaprol, di-methoate, methisazone (— orange— brown), penicillamine... 

Accidental administration of 10 doses of smallpox vaccine to an already vaccinated girl resulted in a clinical picture characterized by neuraxitis with general tonic-clonic convulsions. Complete cure was attained after treatment with hyperimmune antivaccinia globulins and methisazone (38).
