Ladder cage

Not providing a ladder cage or equivalent safeguard for tower access ladders. 

Ladder, cage, and rest platform. These items provide access to the tank deck from groimd level. Occupational Safety and Health Administration (OSHA) requirements dictate the design of this hardware. One requirement to remember is that a rest platform must be provided for every 30 ft of tank height. Therefore, a 32-ft tank will have one rest platform and a 64-ft tank will have two. 

If the ladder is equipped with a ladder cage, employees can strike their head on it while climbing. 

Fixed ladders should have a cage around them so that, were a worker to fall, he or she would fall down, not out. Ideally the bottom exit from the ladder cage should be oriented at 90° from the ladder itself. There have been occasions where a person fell down a ladder, hit the deck, and then fell over the handrail down to the level below.
