Knowledge Assessment

Establish safety education and training for all employees and establish feedback channels to determine whether it is effective along with processes for continual improvement. The education should include reminders of past accidents and causes and input from lessons learned and trouble reports. Assessment of effectiveness may include information obtained from knowledge assessments during audits. 

Continual improvement and feedback to the SUBSAFE training programs comes not only from trouble reports and incidents but also from the level of knowledge assessments performed during the audits of organizations that perform SUBSAFE work. 

A very powerful idea behind Bayesian inference is that statistical inference is simply updating a previous knowledge, assessed by a prior distribution. The obtained posterior distribution, which encodes the current state of knowledge, can be sequentially updated by adding more and more data. To exemphfy this idea, let us consider a very simple problem the Bayesian assessment of an exponential lifetime model, of failure rate X ... 

This knowledge assessment is directed to the following skills. The answers will be provided in Appendix A at the end of the book. A minimum score of 90% in each skill area should be considered acceptable in most cases. However, some skill areas could require a higher score if the risk of failure due to a knowledge gap is high. In aU areas of maintenance, a maintenance person must know the risk.

More detailed analysis of assumption-deviation-risk (Aven 2013a) can be performed by considering 1) the magnitude of a potential assumption deviation, 2) how likely the deviation is (the probabiUty), 3) the consequences given a deviation, and 4) a strength of knowledge assessment of preceding points l)-3).
